Evergreen began with a quiet observation: most primary-care visits in our community last seven minutes. That is not enough time to understand a person, much less the plan they're about to take home. I wanted a practice where the conversation was longer than the prescription.

I'm Stephanie Green, a board-certified Family Nurse Practitioner. I care for adults and families across the lifespan — working parents, military families, retirees, and anyone managing a chronic condition — with the time and attention each visit actually requires.

A 60-minute history, exam, lab review, and shared plan. In person in Peyton, or via secure telehealth across Colorado.

Iterate

Follow-ups as often as your plan needs while we dial things in. Message the practice anytime through the patient portal — adjustments live in your record.

Hold

Once you're stable, we move to wellness and check-in visits and stay in your corner for the long run.
